Anterior instability after primary glenohumeral arthroplasty occurs usually as a result of failure of the subscapularis to heal. Surgery to repair the subscapularis possibly in conjunction with transfer of the pectoralis major, and in some cases with the use of a tendo-achilles allograft can restore stability and control pain, though ultimately functional outcome is frequently marginal.
